The TRT dosage in my book is 100 to 200 mg of testosterone a week, with a blood level of 600 to 1,000 ng/dL as the target. It’s Level 4 on the Natty Spectrum, the hormone pathway of the Forever Protocol, and the one hormone I’ve run for most of the last thirteen years. TRT restores what age takes away, but it also shuts down your own production and your fertility. This page covers the dose, how to know you need it, the labs that keep it safe, and what the biggest heart trial found.

  • The book’s TRT dosage is 100 to 200 mg a week, aiming for 600 to 1,000 ng/dL on bloodwork.
  • Prove it’s low first: symptoms plus two low morning tests. The guidelines put “low” near 300 ng/dL.
  • Hematocrit is the red line. Above 0.52, the book says donate blood. Above 0.60, stop and get help.
  • TRT shuts down your own testosterone and your sperm. Plan for fertility before you start.
  • In the largest trial, TRT didn’t raise heart attack or stroke risk, but it did raise blood pressure and some other risks.

What TRT Is

TRT means testosterone replacement therapy: you take testosterone because your body no longer makes enough. In the US, it’s usually an injection of testosterone cypionate or enanthate. Gels, creams and pellets also exist.

Testosterone drops 1 to 2 percent a year after 30, as I wrote in the opening chapters. For some men, the drop is small. For others, however, it brings low energy, low libido, lost muscle and a foggy head.

The catch is that your brain notices the outside testosterone and stops sending the signal to your testes. So once you start, your own production shuts down. That’s why the book treats TRT as a serious decision.

The TRT Dosage in the Book

The book gives one range for replacement: 100 to 200 mg a week. It shows up as Level 4 of the Natty Spectrum in Chapter 6, in the hormone pathway of the Forever Protocol, and in my 80/20 Protocol “if testosterone is low.” The target on bloodwork is 600 to 1,000 ng/dL.

My own stack, as I describe it in Chapter 6, runs 300 mg a week on top of the Natural Plus foundation. That’s above the replacement range the book gives for everyone else. I’ve also said it plainly in the book: my natural testosterone and my fertility are suppressed, and if I started today, I’d start with the Natural Plus compounds first.

Prove It’s Low First

Before any TRT dosage, prove you need it. The two guidelines the book cites agree on the method [1][2]:

  • Symptoms of low testosterone, like low libido, fatigue or lost muscle.
  • Two morning blood tests, taken on different days, both low.
  • Low means roughly under 300 ng/dL in the guidelines.

After all, one low test after a bad night isn’t a diagnosis. Below are the full TRT dosage table by goal, the hematocrit and estrogen rules, fertility, the heart data, the bloodwork schedule and who should skip it.
